US Coast Guard 719 series Forms

The USCG 719 series forms are used for Merchant Mariner Credential applications. The expiration date on these forms is June 30, 2012. The USCG will continue to accept these forms until further notice. Here is a statement issued by the National Maritime Center on October 18, 2012.

The Coast Guard continues to work through the forms approval process for all of the 719 series forms that merchant mariners use in their application process. The table below lists the forms associated with the mariner credentialing program that has reached their “expiration” date. These forms are still valid and being accepted by the Coast Guard.

The 719 series includes the following forms:

  • 719B – Merchant Mariner Credential application form (expires 30-June-12).
  • 719K and KE – Medical evaluation forms (expires 30-June-12).
  • 719P – USCG/DOT drug screening form (expires 30-June-12).
  • 719S – Small vessel sea service form (expires 30-June-12).
